Member Enrollment
To apply for membership in the Minnesota Academy of Audiology, please download and complete the enrollment form via the link below. Applications should be accompanied by first year's dues and will be reviewed at the next meeting of the Board of Directors (held bi-monthly). Applicants will be notified once membership enrollment is completed. To complete annual membership renewal, members are required to submit a renewal form and the following year's membership dues to the academy offices. An electronic version of the renewal for is supplied below for your convenience. Please feel free to download this form and complete it on your computer (preferred) rather than returning a handwritten version. Members may also log in to the Members' Area for online renewal.
